Key Responsibilities
Ensure continuity of care in inpatient and outpatient settings by
utilizing appropriate resources
Conduct on-site or telephonic reviews of patient care and
coordinate treatment plans
Maintain effective communication with health plans, physicians, and
patients regarding care management
Required Qualifications
Graduation from an accredited Licensed Vocational Nurse program
Current LVN license in California
1+ years of recent clinical experience working as an LVN/LPN
