Speech Therapist Home Visits, Per DiemAtrio Home Health - Grand
Rapids is seeking to add a per diem Speech Language Pathologist to
our team! This home health Speech Language Pathologist works with
assigned clients in their homes to assess and treat speech,
language, swallowing, and cognitive disorders. Successful
candidates will share our passion for providing the highest quality
of care to our clients with the best
outcomes.ResponsibilitiesPerforms speech therapy evaluations to
assess client progress and recommends care plan
adjustmentsDiagnoses, administers, and interprets speech, language,
swallowing, voice, and cognitive disorders using various
testsCompletes OASIS discharges and recertification paperwork per
policyDevelops a treatment plan for clients in consultation with
physicians and care teamCoordinates services in
consultation/communication with other Atrio Home Health
staffProvides speech therapy treatment to assigned
clients.Identifies barriers to desired outcomes and evaluates care
resultsInforms physicians and other personnel of changes in the
client's needsEducates clients and their families on communicative,
cognitive and swallowing functioning and therapeutic
techniquesFunctions as a member of the interdisciplinary team
attending case management meetings and maintaining communication
with other disciplines to facilitate quality client carePrepares
clinical documentation, progress notes, and other required
documentationQualificationsSpeech Language Pathologist License in
MichiganValid Michigan driver's license, proof of auto insurance,
and reliable transportation2 years of experience preferred;
preferably working with the geriatric populationPassion for
