TravelNurseSource is working with Cynet Health to find a qualified Case Manager RN in Detroit, Michigan, 48202 About The Position Job Title: Manager Job Specialty: Integrated Case Job Duration: 14 months Shift: Full-Time Day Shift (8:30 AM - 5 PM), rotating schedule with weekend availability required Guaranteed Hours: 40.00 hours per week Experience: 3-5 years of case management experience License: - Registered Nurse (RN) with a valid, unrestricted State of Michigan license. OR - Licensed Social Worker (LMSW) with a valid, unrestricted State of Michigan license. Certifications: N/A Must-Have: - Clinical background - Strong computer skills - Strong communication and interpersonal skills - Experience in case management Job Description: - Conduct comprehensive assessments of patient and family/caregiver biomedical, psychological, social, and functional needs to evaluate potential impacts on recovery. - Develop personalized, patient-centered care plans aimed at optimizing the patient's care experience. - Engage patients and their families as part of the care team through advocacy, communication, health education, resource identification, and service facilitation. - Utilize professional judgment, critical thinking, motivational interviewing, and self-management techniques to assist patients in overcoming barriers to goal achievement. - Provide counseling and interventions related to treatment decisions and end-of-life issues, including advanced care planning. - Ensure seamless and safe transitions for patients between care settings. - Advocate for appropriate delivery of services within the patient's health plan benefit structure. - Collaborate with treatment/care team members to co-manage patients with complex medical and social needs, facilitating interdependent collaborative care conferences. - Continually evaluate patient responses to the care/treatment plan, making necessary modifications. - Plan and participate in process improvement activities designed to reduce risk, including data collection, analysis, and follow-up intervention activities. - Facilitate interventions in cases involving child abuse and neglect, domestic violence, elder abuse, institutional abuse, and sexual assault. - Support department-based goals contributing to organizational success. - Perform other duties as assigned.
